Office forms are needed for your initial visit. The information required is reduced to a minimum.

(For pilot studies with physicians, we have a different set of questionnaires which are provided separately).

 

For the INITIAL visit, you have ONE of two choices:

1. Click on the links below and
   • Print the resulting PDF files from your browser
   • Complete the forms and bring them to your visit
2. Arrive early and complete your information at the office

For ALL your visits:

1. Please come well hydrated as results are better with well hydrated clients
2. Wear comfortable form fitting clothes, preferably of natural fibers
3. Wear clean socks
4. Purchase a new toothbrush for when you get home
